How to Apply & Schedule Installation

1. Complete and submit the intake form below.

2. Select your preferred installation date (at least two weeks from intake form submission).

3. Once your application is reviewed and approved, you will receive a confirmation email from The Updated Bulletin (TUB) with your official installation date and next steps.

📌 Important: Please add TUB to your contacts to ensure you receive all confirmation and scheduling emails without delay.
